Patients who have been asked by a Healthcare professional to have a blood test can book an appointment online at the Mersey Care – Book My Blood Test page:
Book a blood test in Southport and Formby
Appointments are available at:
- Ainsdale Centre for Health and Wellbeing
- Churchtown Clinic
- Formby Clinic
- Southport Centre for Health and Wellbeing
Patients will need their NHS number to complete the online booking. They should also bring their blood test request form and their booking confirmation email or text message to the appointment.
Patients who cannot book online can telephone the booking team on 0151 285 4548. Urgent blood tests must be arranged by the GP practice
